He thanks someone who cares for him without being part of his family, a nurse perhaps. This makes me think that many times we find in other people what our own family doesn't give us; in fact, perhaps they give us the complete opposite, and it's better to distance ourselves.

Or the woman holding the child. What is on the painting are flashes of his life.
I believe you are right. My grandmother had a stepmother since her mother died and only have good thoughts about her. Unfortunately, my mother is not the one I have good thoughts about, on the contrary.
